The document title and the permissions they ticked.

Name the document. Do not give advice.
A consent or release reply names the document on the form. It does not explain the person's rights or add a permission they did not tick.
Thanks Noel. We have the photo release you signed, and only the uses that were ticked. This note does not add a further permission.
